The International Society of Minimally Invasive and Virtual Surgery (ISMIVS, previously known as International Society of Minimally Invasive and Noninvasive Medicine) was founded in July 2013 at the gala of the 1st Yangtze International Summit of Minimally Invasive and Noninvasive Medicine in Chongqing, China. ISMIVS envisions to accelerating the progress in minimally invasive and noninvasive medicine by promoting research, education, communication, and international collaboration.

The Personality in the Medical Education World 医学教育界的大人物
From the 2020 July issue of our ISMIVS Newsletter, we would like to introduce some pioneers in the field of minimally-invasive and noninvasive medicine. They are not only prestigious in their field but also willing to share their experience and ideas with younger doctors, hence making a great contribution to medical education. This issue features Dr. Orawin Vallibhakara from Ramathibodi Hospital, Mahidol University, Thailand, who is a specialist in the field of gynaecology and obstetrics.

Dr. Orawin Vallibhakara is the Head of the Menopause Unit, Obstetrics and Gynecology Department, Faculty of Medicine, Ramathibodi Hospital, Mahidol University. She got her diploma in Reproductive Medicine from the Faculty of Medicine Ramathibodi Hospital of Mahidol University in 2014, and her diploma in Sexual Medicine from the Faculty of Medicine of Thammasat University in 2023.
Orawin Vallibhakara 博士是玛希隆大学拉玛提博迪医院医学系妇产科更年期科主任。她于 2014 年获得玛希隆大学拉玛提博迪医院医学系生殖医学文凭,并于 2023 年获得塔玛萨特大学医学系性医学文凭。
She is an experienced gynecologist with a special interest in epidemiology and biostatistics, disease outbreak and surveillance, ageing research of menopause, women's health, and wellness, osteoporosis, sarcopenia and obesity.

As the Standing Member of the Young ISMIVS Committee, Dr. Orawin Vallibhakara is currently digging into high-intensity focused ultrasound, an innovative noninvasive technology, and is inspired to treat women with gynecological disorders in a harmless way.

By being actively involved in academic conferences and seminars, such as being the guest speaker on “Common dietary & supplements for postmenopausal musculoskeletal Health”, and moderator on “Supplements: The spare key for sleepless menopause”, she has done a lot of work to raise the awareness for women’s healthcare and wellbeing.

How to manage liver cancer with high-intensity focused ultrasound?

On July 9th, 2023 Treatment strategies for patients with liver tumors, one section of the 5th Yangtze International Summit of Minimally Invasive and Noninvasive Medicine & 2023 “One Belt One Road” Innovation Forum on Minimally Invasive and Noninvasive Medicine was successfully held.
2023 年 7 月 9 日,“第五届国际微无创医学长江高峰论坛暨 2023 ‘一带一路’微无创医学创新论坛”分论坛“肝癌的治疗策略”圆满举行。
This section was co-chaired by Professor Mohamed Hamed of Cairo University Affiliated Hospital, Egypt, Xu Ning, Director of the Oncology Department of Fenyang Hospital, Shanxi Province, and Li Yongjie, Director of the Second Ward of the Oncology Department of Hezhou People's Hospital. Five experts gave brilliant lectures on the treatment strategies for liver cancer, discussed the cutting-edge treatment options for liver cancer, how to choose between different treatment options for liver cancer patients in different conditions, and what to do with cases unable to receive surgery.
本分论坛由埃及开罗大学附属医院教授 Mohamed Hamed、山西省汾阳医院肿瘤科主任许宁、贺州市人民医院肿瘤科二病区主任李永杰共同担任主持。共计 5 位专家围绕“肝癌的治疗策略”进行精彩授课分享,共同探讨肝癌的最新治疗方案,不同病情肝癌患者该如何选择手术方案,对不能手术的肝癌又该何去何从。
Yang Tong, Chief Physician of Ningbo Second Hospital, shared a lecture on the expectation of immunology treatment for solid tumor treated by HIFU. HIFU could solve two problems in the treatment of solid tumor: complex tumor driver genes and diverse clonal origins. Specific antigens are difficult to find, and 1-2 specific CAR-T or DC cells do not solve the problem. The fortress problem is TME(tumor micro-environment). It is expected that HIFU could bring about a revolutionary change to Abscopal. In general, HIFU is a new choice for patients with malignant solid tumor.
宁波市第二医院主任医师杨峂分享“HIFU在实体肿瘤免疫治疗中的期待”的专题讲座。HIFU在实体瘤治疗同时解决了 2 个问题:肿瘤驱动基因复杂,克隆起源多样,特异性抗原难寻,1-2 种特异性的 CAR-T 或者 DC 细胞不解决问题;堡垒问题:TME(tumor micro-envirement)。期待 HIFU 带来 Abscopal 的革命性变化。综合而言,HIFU是恶性实体肿瘤患者的一种新选择。
Professor Mohamed Hamed from Cairo University Hospital, Egypt discussed HIFU in liver tumours, where do we stand? - Egyptian experience. He introduced the condition of HIFU treatment for liver cancer, which is not less effective than surgery while reducing the patients’ pain. Even patients who lose the chance of surgery can be treated by HIFU.
埃及开罗大学附属医院教授 Mohamed Hamed 分享“HIFU 治疗肝脏肿瘤,我们的情况如何?——来自埃及的经验”的专题讲座。他介绍了 HIFU 在肝脏肿瘤的治疗情况,有不低于手术的有效性,同时减轻了患者的痛苦,甚至是失去手术机会的患者,也可以用 HIFU 治疗。
Feng Gang, Deputy Chief Physician of Mianyang Central Hospital, shared his views on HIFU in the application of hepatic malignancies. HIFU’s application in the treatment of liver cancer includes primary liver cancer, intrahepatic cholangiocarcinoma, and liver metastases. Meanwhile, HIFU has contributed to the disciplinary construction since it is a unique non-invasive therapy, does not affect the progress of other treatments (chemotherapy, targeting, immunity), and improves the curative effect of medical oncology treatment as a whole.
绵阳市中心医院副主任医师冯岗分享“HIFU 在肝脏恶性肿瘤中的应用” 的专题讲座。HIFU 在肝脏肿瘤中的应用包括:原发性肝癌、肝内胆管癌、肝转移癌,此外,HIFU对学科建设的贡献,独有无创治疗技术,不影响其他治疗(化疗、靶向、免疫)的进程,整体提升肿瘤内科治疗疗效。
Franco Orsi, Professor of the European Institute of Oncology, Italy, discussed percutaneous ablation for CRLM: technique and outcome. He gave a brilliant explanation of the minimally invasive treatment of tumor, and shared cases of percutaneous ablation for CRLM. Survival was comparable in the “ablation + partial hepatectomy group” to the “partial hepatectomy group”, potentially leading to long-term disease control with a lower complication rate. Thus ablation is favored over chemotherapy alone. At the same time, combining thermal ablation with chemotherapy reduces morbidity, mortality, and costs, and improves survival.
意大利欧洲肿瘤研究所教授 Franco Orsi 分享“经皮消融治疗结直肠肝转移(CRLM):技术与疗效”的专题讲座。他对肿瘤领域的微创治疗进行了精彩讲解,并分享了经皮消融治疗结直肠肝转移的病例,消融+部分肝切除组与单纯部分肝切除组的存活率相当,有可能导致长期的疾病控制,而且并发症发生率较低,支持消融而不是单纯化疗。同时在化疗的基础上加用热消融术可以降低发病率、死亡率和成本,提高生存率。
Wu Mingsong, Director/Chief Physician, Department of Oncology and Hematology, Beiliu People's Hospital, discussed clinical practice in the minimally invasive and non-invasive treatment of hepatobiliary and pancreatic cancer guided by integrated medical sciences. Initial progress has been made in the integrated medical sciences of hepatobiliary and pancreatic cancer, which deserves further exploration. Integrated medical sciences is an emerging medical system emphasizing “disease” to “patient”, embodying humanistic medicine. Minimally invasive intervention plus HIFU non-invasive ablation, as well as organic integration of other systemic treatments and conditioning, can benefit patients more.
北流市人民医院肿瘤血液内科主任/主任医师吴明松分享“整合医学模式下微无创治疗在肝胆胰恶性肿瘤中的临床实践”的专题讲座。整合医学诊疗体系在肝胆胰恶性肿瘤方面初见成效,值得进一步探索。整合医学是新兴的医学体系,强调从“病”到“人”,体现人文医疗。微创介入+HIFU 无创消融,以及其他的系统治疗、调理等有机整合,更能使患者获益。

HIFU Special Issue 高强度聚焦超声特刊

We are delighted to announce that the International Journal of Hyperthermia, the HIFU Special Issue (i.e. Volume 38, 2021), which was partially supported by ISMIVS, was officially released in September. The Special Issue includes 15 articles written and reviewed by world-renowned scholars and pioneers in minimally invasive and noninvasive medicine, elaborating on the status and prospects of the clinical applications of high intensity focused ultrasound on tumours that occurred in specific organs. We'll share the 15 articles included in the upcoming newsletters, stay tuned!

Oxford's clinical experience in the development of high-intensity focused ultrasound therapy

Oxford has been a centre for both clinical and preclinical research in HIFU over the last twenty years. Research into this technology in the UK has a longer history, with much of the early research being carried out by Professor Gail ter Haar and her team at the Institute of Cancer Research at Sutton in Surrey. A broad range of potential applications have been explored extending from tissue ablation to novel drug delivery.

This review presents Oxford’s clinical studies and applications for developing this non-invasive therapy. This includes treatment of solid abdominal tumours comprising the liver, kidney, uterus, pancreas, pelvis and prostate. It also briefly introduces preclinical and translational works that are currently being undertaken at the Institute of Biomedical Engineering, University of Oxford.

The effect of virtual reality on pain and anxiety management during outpatient hysteroscopy: a systematic review and meta-analysis of randomized controlled trials

A systematic review published by the author from Saudi Arabia aimed to evaluate the effectiveness of virtual reality (VR) intervention for pain and anxiety relief during outpatient hysteroscopy has come to our attention.

Six RCTs were retrieved, involving a total of 457 patients. They used Revman software to perform our meta-analysis. The Visual Analog Scale (VAS) was used to assess pain and anxiety.

Virtual reality was associated with a significant reduction in pain score during the procedure in comparison with the control group (MD = − 1.43, 95% CI [− 1.69, − 1.16], p < 0.001). In addition, there was a significant decrease in anxiety during the procedure among the virtual reality group compared to the control group (p = 0.01).

On the whole, Virtual reality technology is a simple, feasible, and effective intervention for reducing pain and anxiety during outpatient hysteroscopy. More trials are required to confirm our findings.
