The International Society of Minimally Invasive and Virtual Surgery (ISMIVS, previously known as International Society of Minimally Invasive and Noninvasive Medicine) was founded in July 2013 at the gala of the 1st Yangtze International Summit of Minimally Invasive and Noninvasive Medicine in Chongqing, China. ISMIVS envisions to accelerating the progress in minimally invasive and noninvasive medicine by promoting research, education, communication, and international collaboration.

The Personality in the Medical Education World 医学教育界的大人物
From the 2020 July issue of our ISMIVS Newsletter, we would like to introduce some pioneers in the field of minimally-invasive and noninvasive medicine. They are not only prestigious in their field but also willing to share their experience and ideas with younger doctors, hence making a great contribution to medical education. This issue features Prof. Milka Marinova, a leading specialist in Radiology from the University Hospital Bonn, Germany.

Professor Milka Marinova, a distinguished physician and specialist in radiology, currently works at the University Hospital Bonn in Germany. Having earned doctoral degrees in both, Human Medicine and Pharmaceutical Science, from the University of Bonn, she started her professional career in 2012.

As a leading physician and researcher, Prof. Marinova has specialized in tumor management including the innovative, non-invasive treatment of solid tumors with high-intensity focused ultrasound (HIFU). She supervised more than 20 doctoral theses in the field of medicine and tumor therapy and has received funding from prestigious programs for clinical trials. She has served as a reviewer for numerous international, peer-reviewed journals. Her outstanding contributions have been recognized with awards such as the DEGUM Science Award 2017 and the best scientific abstracts at Interventional and Ultrasound Congresses. These awards commend her profound work in the field of high-intensity focused ultrasound (HIFU), particularly in the treatment of pancreatic cancer.

Since 2015, Prof. Milka Marinova has been at the forefront of the clinical application of HIFU, using this innovative technology to revolutionize the management of pancreatic cancer and other solid tumors. Her extensive clinical research and profound expertise have led to the publication of numerous articles in this specialized field. These publications have contributed significantly to the broader clinical application of HIFU and the establishment of common standards in this field.

On July 8th, Minimally Invasive and Noninvasive Medicine Empowers the High-Quality Development of Hospitals, one section of the 5th Yangtze International Summit of Minimally Invasive and Noninvasive Medicine & 2023 “One Belt One Road” Innovation Forum on Minimally Invasive and Noninvasive Medicine was successfully held.

Wang Zhibiao, Director of the National Key Laboratory of Ultrasound in Medicine and Engineering, delivered a special lecture on "The Future and Challenges of Minimally Invasive Medicine." By fully integrating big data, artificial intelligence, and information technology, noninvasive diagnosis and treatment, including digital anatomy, diagnosis and surgery can be achieved. He proposed the concept of parallel operating rooms based on the digitalized focused ultrasound ablation surgery, which makes it possible to perform surgeries across time and space.

He Jia, Vice Chairman of the Suining Municipal Political Consultative Conference and former Dean of Suining Hospital of Traditional Chinese Medicine affiliated with North Sichuan Medical College, shared a lecture on Experience Sharing: Focused Ultrasound Ablation Technology Boosting the Development of Municipal Hospitals. The lecture highlighted that given the fact that there exists a significant disparity in terms of talent, academic resources, and economic capabilities between hospitals in municipal-level cities and nationally-funded institutions, to bridge this gap and surpass the achievements of institutions with overwhelming advantages, hospitals and departments must identify precise technical directions. By doing so, they can strive to catch up with and even surpass these top-tier institutions. The hospital should identify its areas of strength and advantage in various disciplines, clearly define its development direction, establish a comprehensive hospital platform, and cultivate unique competitive edges. By leveraging distinctive technologies, the hospital can enhance its visibility and competitiveness. The support at the institutional level, combined with the collaborative efforts of departmental teams, will warrant the high-quality development of the hospital.
遂宁市政协副主席/原川北医学院附属遂宁中医医院院长何佳分享“聚焦超声消融技术助力地市级医院发展经验分享”的专题讲座。总结地市级医院的人才、学术、经济资源不能与“国家队”相比,医院、科室只有找到精准的技术方向,才有可能实现“弯道加速”甚至“弯道超车”医院确定优势学科,明确发展方向,建立医院平台,形成医院特色。依托特色技术,扩大医院影像力,提升医院竞争力。院级层面的支持,科室团队的合力,必然助力医院高质量发展。

Guo Qing, Deputy Secretary of the Party Committee and President of Shijiazhuang Obstetrics and Gynecology Hospital delivered a special lecture on Promoting High-Quality Hospital Development through Discipline Construction. Promoting high-quality development of the hospital entails defining clear development goals for discipline and making efforts to build disciplinary teams, collaborating with the best teams. Efforts also must be made to take the project as the starting point to improve the management concept of the whole hospital, to lead the quality of obstetrics services in the whole province with the quality control center and to integrate clinical and scientific research to yield mutual development and leap forward.
石家庄市妇产医院党委副书记、院长郭清分享“以学科建设促进医院高质量发展”的专题讲座。推动医院高质量发展,需明确学科发展目标,全力打造学科团队,与最优秀团队合作,以项目为抓手提升全院管理理念,以质控中心来引领全省产科质量,临床与科研结合与转化互相促进。

Lung Chengyu, Director of OBS/GYN of Kaohsiung Medical University Hospital, shared his views on Technological Innovation and Discipline Construction - High-Intensity Focused Ultrasound and Lower urinary Symptoms Control. After uterine fibroids are successfully ablated and reduced, a large number of lower urinary tract symptoms can be reduced.
高雄医学大学附设医院妇产科主任龙震宇分享“技术创新与学科建设-高强度聚焦超声与下尿路症状控制”的专题讲座。子宫肌瘤被成功消融缩小后,可减少大量下尿路症状。

Xia Menghong, President and Chief Physician of Nanchuan District People's Hospital in Chongqing discussed on "Ultrasound + Medical Research Platform: Empowering High-Quality Development of Public Hospitals." The lecture emphasized that the key to high-quality hospital development lies in the word “High”, which could be reflected by whether the predetermined tasks and goals were reached, in-depth data demonstrated, significant breakthroughs and meeting high standards. Characteristic development ideas can be formulated through considering both improvement in quantity and quality, and promoting the high-quality integrated development of “Ultrasound plus Hospital”. Furthermore, cross-disciplinary cooperation is strengthened between focused ultrasound and artificial intelligence to realize a condition that the whole is greater than the sum of its parts, fostering interdisciplinary development strategies.
重庆市南川区人民医院院长/主任医师夏孟红分享“超声+医学研究平台助力公立医院高质量发展”的专题讲座。医院高质量发展,实质在“高”,主要在于是否完成既定任务,达到预定目标,展现高内涵数据,实现重大突破、体现高标准。从“量的提升”和“质的发展”综合考量,利用超声+助力医院一体化高质量融通发展,形成特色发展思路;利用加强聚焦超声与人工智能跨领域合作,实现 1+1>2 的效果,形成跨学科发展思路。

Yoon Chonghyeak, founder and chief director of Angelot Women's Hospital in South Korea, shared a special lecture on "How to Make Our Hospital Stand Out." In Seoul, over 2,000 patients with uterine fibroids and adenomyosis have undergone non-invasive focused ultrasound ablation surgery throughout the year. South Korean women attach great value to their appearance. As focused ultrasound ablation surgery features scar-free, fast recovery and minimal impacts on appearance, an increasing number of Korean women choose focused ultrasound ablation surgery as an alternative to traditional surgery.
韩国Angelot女性医院妇科主任尹钟赫分享“如何使我们的医院与众不同”的专题讲座。在首尔地区,全年有超过2000 名子宫肌瘤和子宫腺肌症患者接受了无创的聚焦超声消融手术。韩国女性对外观较为注意,完成聚焦超声消融手术后无疤痕,加快患者复原及减低对外观影响,以致越来越多韩国女士选择用聚焦超声消融手术替代传统手术。

Li Jidong, Secretary of the Party Committee and Chief Physician of Orthopaedics of Shijiazhuang Western Medical Center (Jingxing County Hospital), shared a special lecture on "Minimally Invasive Medicine Promoting High-Quality Development of County-Level Hospitals in China." The lecture elaborated on hospital conditions, minimally invasive intervention centers, emphasis on talent cultivation and competence training, and minimally invasive techniques. He emphasized on the concept of "minimizing harm to patients," which permeates all aspects of hospital construction and development. The goal is to establish a medical centre where people can receive good, affordable, and trustworthy healthcare services.
石家庄市西部医疗中心(井陉县医院)党委书记、骨科主任医师李计东分享“微无创医学助推中国县级医院高质量发展”的专题讲座。通过医院情况、微创介入中心、重视人才培养及技术培训、微无创技术等方面,详细阐述了“让患者受伤害更小”的微无创理念无不贯穿于医院建设发展的方方面面,要建立老百姓看得好病、看得起病、放心选择的医疗中心。

Zhao Shuping, Director and Chief Physician of Gynecology Center of Women and Children's Hospital, Qingdao University, shared a special lecture on "Technological Innovation and Discipline Development: Discussing the Selection of Treatment Methods for Adenomyosis-Related Infertility." The lecture provided a comprehensive overview of the impact of technological innovation and discipline development on hospital construction, focusing on case studies, the mechanisms of infertility caused by adenomyosis, treatment options for adenomyosis, and fertility guidance.
青岛大学附属妇女儿童医院妇科中心主任/主任医师赵淑萍分享“从子宫腺肌病合并不孕治疗方式的选择谈技术创新与学科发展”的专题讲座。从病例分享、子宫腺肌病导致不孕的机制、子宫腺肌病治疗选择与生育指导等方面综合介绍技术创新与学科发展对促进医院建设的影响。

Ying Zonghe, President of Chung Shan Medical University in Taiwan, shared a special lecture on "Comparative Analysis of Psychological State, Symptom Distress, and Quality of Life in Patients with Uterine Fibroids Undergoing High-Intensity Focused Ultrasound and Myomectomy." The lecture focused on the psychological state and symptom distress of patients before and after treatment, as well as their impact on quality of life. Symptom distress was found to be a more effective predictor of quality of life than negative psychological state. Both groups showed a significant negative correlation between symptom distress and quality of life, regardless of the study's cross-sectional or longitudinal design. However, in the high-intensity focused ultrasound group, the impact of a negative psychological state on quality of life was more pronounced compared to the surgical myomectomy group. Therefore, it is important to pay attention to the negative psychological state of women undergoing high-intensity focused ultrasound treatment.
台湾中山医学大学院长应宗和分享“子宫肌瘤患者接受高强度聚焦超声和肌瘤切除术的心理状态、症状困扰与生活品质之比较分析”的专题讲座。治疗前的负面心理状态和症状困扰对治疗后的生活品质是至关重要的影响因素。症状困扰比负面心理状态更能有效预测生活品质。无论在横断及纵贯情况下,两组的症状困扰都能与生活品质有显著的负相关。在海扶刀组,负向心理状态对生活品质之影响,较手术切除组更为明显,故需对海扶刀组妇女之负向心理状态给予关注。

Wang Sangui, Secretary and President of the General Party Branch of Dongguan Hepatobiliary Hospital, shared a special lecture on "Exploration and Practice of the Minimally Invasive Concept." The lecture highlighted the distinctive techniques of bile preservation surgery and focused ultrasound ablation surgery, which complement each other and contribute to the future of the hospital's specialized medical technologies. It emphasized that no trend can hinder the progress and development of the hospital's characteristic medical techniques.

HIFU Special Issue 高强度聚焦超声特刊


We are delighted to announce that the International Journal of Hyperthermia, the HIFU Special Issue (i.e. Volume 38, 2021), which was partially supported by ISMIVS, was officially released in September. The Special Issue includes 15 articles written and reviewed by world-renowned scholars and pioneers in minimally invasive and noninvasive medicine, elaborating on the status and prospects of the clinical applications of high intensity focused ultrasound on tumours that occurred in specific organs. We'll share the 15 articles included in the upcoming newsletters, stay tuned!

High-intensity focused ultrasound (HIFU) for the treatment of uterine fibroids:
does HIFU significantly increase the risk of pelvic adhesions?
高强度聚焦超声(HIFU)治疗子宫肌瘤: HIFU 治疗是否会增加盆腔粘连的风险?

This article aims to investigate the prevalence of pelvic adhesions in patients with uterine fibroids after high-intensity focused ultrasound (HIFU) treatment, and then analyze the influencing factors of pelvic adhesions.
本文旨在调查子宫肌瘤患者接受高强度聚焦超声(HIFU)治疗后盆腔粘连的情况,并分析盆腔粘连的影响因素。
From October 2010 to March 2020, a total of 1809 patients with uterine fibroids underwent either hysterectomy, myomectomy, or cesarean section in Suining Central Hospital of Sichuan Province. Among them, 96 patients with uterine fibroids had had prior HIFU treatment (HIFU group), and 1713 patients had not had HIFU or surgical treatments (control group).
2010 年 10 月至 2020 年 3 月,四川省遂宁市中心医院共有 1809 例子宫肌瘤患者接受了子宫切除、肌瘤切除或剖宫产手术。 其中,96 例子宫肌瘤患者既往接受过 HIFU 治疗(HIFU组),1713 例患者未接受过 HIFU 或手术治疗(对照组)。
Among the 96 patients in the HIFU group, adhesions were detected in 42 patients, with the incidence of pelvic adhesion being 43.75%; in the 1713 patients in the control group, adhesions were detected in 619 patients, the prevalence of pelvic adhesion was 36.14%. No statistically significant difference in the incidence of adhesion between the two groups was observed.
HIFU组 96 例患者中,盆腔粘连 42 例,盆腔粘连发生率为 43.75%; 对照组 1713 例患者中,盆腔粘连 619 例,盆腔粘连患病率为 36.14%。 两组粘连发生率无统计学差异。
Based on the results with limited numbers, it had been concluded that HIFU treatment did not significantly increase the risk of pelvic adhesions.
基于以上结果得出结论:HIFU治疗不会增加盆腔粘连的风险。

Retrospective review of endometriosis surgery at Te Whatu Ora – Capital and Coast

Recently, a retrospective cross-sectional study of endometriosis surgery at Te Whatu Ora - Capital and Coast published by authors from New Zealand aroused our attention.

There were 436 surgeries for suspected endometriosis performed during 2018 and 2019, and endometriosis was diagnosed in 68.3% of these surgeries. One of their goals is to describe the number of repeat laparoscopies performed, and how many people had endometriosis present at their repeat surgery.

They found that around two-thirds of people who have surgery for suspected endometriosis subsequently have evidence of disease, predominantly of the pelvic peritoneum, with the majority being histologically confirmed. Most people having surgery reported pain symptoms and around a fifth reported infertility. Only a small proportion of the total surgeries were repeat laparoscopies.
他们发现,大约三分之二因疑似子宫内膜异位症而接受手术的患者随后有疾病的迹象,主要表现为盆腔腹膜,其中大多数是经组织学证实的。大多数接受了手术的患者反馈有疼痛症状,大约五分之一的患者反馈为不孕。只有一小部分手术需要再行腹腔镜手术。
