The International Society of Minimally Invasive and Virtual Surgery (ISMIVS, previously known as International Society of Minimally Invasive and Noninvasive Medicine) was founded in July 2013 at the gala of the 1st Yangtze International Summit of Minimally Invasive and Noninvasive Medicine in Chongqing, China. ISMIVS envisions to accelerating the progress in minimally invasive and noninvasive medicine by promoting research, education, communication, and international collaboration.

The Personality in the Medical Education World 医学教育界的大人物
From the July issue of our ISMIVS Newsletter, we would like to introduce some pioneers in the field of minimally-invasive and noninvasive medicine. They are not only prestigious in their field but also willing to share their experience and ideas with younger doctors, hence making a great contribution to medical education. In this issue, we are going to introduce Professor Philippe J. Descamps from France.

Prof. Philippe Descamps is Head of the Obstetrics and Gynaecology Department at the University Hospital, Angers, France. He has also held the role of Chairman of the department since 1999. His scientific interests include gynaecologic endoscopic surgery (specifically genital prolapse, fibroids, urogynaecology and endometriosis) and gynecologic cancers (endometrial, cervical and ovarian cancers).

Prof. Philippe Descamps has been a lecturer in laparoscopic, vaginal and oncologic surgery to an audience of fellows, residents, medical students, midwives, nurses in Gynecology and Obstetrics at the University Hospital, Angers, France. He has also undertaken numerous teaching and surgical programs for CNGOF, including holding courses in France (6-8 surgical workshops in Angers since 2008 (laparoscopy and vaginal surgery), Vietnam, Cambodia, Laos, Mali, Brazil and Bosnia.

ISMIVS launching its online training program
The International Society of Minimally Invasive and Virtual Surgery is pleased to announce the availability of a series of online training courses on the clinical application of noninvasive focused ultrasound therapy. A variety of courses related with ultrasound-guided high intensity focused ultrasound (HIFU) ablation of uterine fibroids will enlighten doctors about the physics and biological effect of HIFU, case screening, clinical protocol, anesthesia plan, nursing care, etc. In the meantime, more courses on focused ultrasound therapy of other diseases will be uploaded in the future, please follow us on www.ismivs.org for what you are interested in.

Online course: HIFU Treatment of Uterine Fibroids: Clinical Protocol
This lecture shall introduce the therapeutic principles, indications and contraindications, and dos and don'ts before, during and after HIFU therapy of uterine fibroids. Through this lecture, trainees are expected to understand the whole process of HIFU therapy of uterine fibroids, know how to screen and prepare paitents for HIFU therapy, determine the treatment time, evaluate the therapeutic effectiveness, and know the criteria for termination of treatment and the nursing care for HIFU therapy, etc.
Online course: Case Study on HIFU Ablation of Uterine Fibroids
Following the lecture above, this video recording presents the process of HIFU ablation of multiple uterine fibroids. Through this video, trainees are expected to understand how HIFU ablation of fibroids is performed on a HIFU therapy machine.

Gynecological surgeries during COVID-19 pandemic: A laparoscopist's viewpoint
This report aims to identify the optimum route of gynecological surgeries in times of COVID-19 which is safe for health-care workers and results in a better patient outcome.
The risk of presence and transmission of SARS-CoV-2 via surgical smoke and pneumoperitoneum is merely theoretical, and there is no such evidence. Furthermore, the advantages of laparoscopy such as early postoperative recovery, short hospital stay, and less complications will result in less exposure of health-care workers and patients than open surgery. The smoke generated in laparoscopy remains contained and can be evacuated through smoke evacuators or filtration system. While, it is not possible in open surgeries and the risk of exposure is high.
There are no data available for hysteroscopy and its safety in COVID era. Considering the fact that SARS-CoV-2 is not documented in the female genital tract, and general anesthesia is not necessary, hysteroscopy appears to be safe.

In conclusion, laparoscopy should be preferred over open surgery wherever feasible.

A comparison of reproductive outcomes of patients with adenomyosis and infertility treated
with High-Intensity focused ultrasound and laparoscopic excision
A retrospective study compared the treatment effects of of high-intensity focused ultrasound (HIFU) and laparoscopic excision (LE) in 93 patients with adenomyosis and infertility, 50 with HIFU and 43 with LE.
Both HIFU and LE treatments achieved significant relief of dysmenorrhea and menorrhagia. The total hospital stay was shorter in patients treated with HIFU than in those who underwent LE surgery. Neither HIFU nor LE treatment led to severe complications after treatment. Most importantly, patients treated with HIFU showed significantly higher pregnancy rates and natural conception rates than those who underwent LE surgery. Notably, in the HIFU treatment group, those with diffuse adenomyotic lesions had significantly lower postoperative pregnancy rates than those with focal adenomyosis.

HIFU showed a safe and effective profile as a therapeutic management option for patients with adenomyosis. In comparison with LE, HIFU treatment achieved better postoperative reproductive outcomes.

Joint RCOG/BSGE Guidance on Gynaecological Endoscopy during the COVID-19 Pandemic

The RCOG and Specialist Societies have developed a range of guidance for gynaecological services during the COVID-19 pandemic, including the Joint ROCG/BSGE Guidance on Gynaecological Endoscopy published on 21 December, 2020. This joint guidance provides a framework for the care of women undergoing emergency and elective laparoscopic or hysteroscopic surgery during the evolving COVID-19 pandemic.
Best practice should be followed for diagnostic and operative hysteroscopy procedures to minimise the risk of complications requiring further health care interventions, general contamination from blood, urine, genital tract fluids and faeces, etc.
Laparoscopic surgery should be selected in keeping with usual, best practice. For example, the port positioning and instrument choice for gynaecological laparoscopic operations should be according to the surgeon and hospitals usual practice to minimise time in theatre and the risk of operative complications; suction devices, smoke evacuation filters, retrieval devices and swabs should be used to prevent aerosol transmission and potential droplet transmission.
腹腔镜手术的选择应遵循通常的最佳临床实践做法。 例如,妇科腹腔镜手术的port放置和仪器选择应根据外科医生和医院的常规做法,以尽量减少在手术室的时间和降低手术并发症的风险; 应使用抽吸装置、烟雾疏散过滤器、回收装置和棉签,以防止气溶胶传播和潜在的液滴传播。
